Privacy Policy

Who we are

Assist Healthcare Pty Ltd (ABN 56 664 874 855) provides physiotherapy and allied health in clients' homes across greater Adelaide and greater Melbourne. As a health service provider we are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles, and for our Victorian clients by the Health Records Act 2001 (Vic).

What we collect

To assess and treat a client, we collect their name, contact details, date of birth, address, funding type, the reason for referral, relevant medical history, diagnosis and known allergies. This is health information, and we treat it with the extra care the law requires.

We also collect the name, organisation, phone number and email of the person who refers a client, and the details people give us when they contact us through this website.

How we collect it

Most client information comes from the care partner at the client's home care provider, through our referral form, by email or by phone. The referrer confirms that the client, or their representative, has agreed to the referral. We also collect information from clients and their families directly, and during visits.

How we use it

We use client information to assess and treat the client, to report progress to their care partner, to arrange visits, to bill the right funding source, and to meet our legal and professional obligations. We do not use it for marketing, and we do not sell it.

Who we share it with

We share information only with the clinicians involved in the client's care, the client's care partner and home care provider, the funding body where billing requires it, and where the law requires or allows it.

Services that store information for us

We use service providers to run our email, website and records. They include Microsoft 365 for email, Wix, which runs this website and stores the forms sent through it, including referrals, on servers that may be outside Australia, and our clinical records system. Where information is held outside Australia, we take reasonable steps to make sure it is protected to the standard of the Australian Privacy Principles.

This website

When you send a referral or a message through this website, we collect the details in the form and use them only for that referral or to reply. This website runs on Wix, which uses cookies the site needs to work and collects basic visit statistics. It does not use advertising cookies, and we do not track you across other websites.

How we store and protect it

Information is kept in systems with restricted access, and only staff who need it for a client's care can see it. We keep health records for the periods the law requires and then destroy or de-identify them.

Accessing and correcting your information

You can ask to see the information we hold about you, and ask us to correct it. Contact us using the details below. We will respond within 30 days.

Making a complaint

If you are concerned about how we have handled your information, contact us first so we can try to resolve it. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner (oaic.gov.au). Victorian clients can also contact the Health Complaints Commissioner (hcc.vic.gov.au), and South Australian clients the Health and Community Services Complaints Commissioner (hcscc.sa.gov.au).
